How Do You Track Behavior?

Ah, classroom management.  It's one of the hardest things to keep track of in the classroom.  I use this form to help me remember what rule has been broken by a student.  There are codes at the bottom of the form, so I only spend about a second jotting down the "offense".  Who has more time than a second?  At the end of the day, I use this from to write a note in the student's agenda.  This form make my life a lot easier.  I hope you can use it.
